This case documents the implant-based rehabilitation of the upper jaw through the combined use of conventional and trans-sinus implants, performed by Dr. Luca Palumbo. The objective was to restore masticatory function in a patient with partial edentulism and reduced posterior bone volume.

Following thorough preoperative clinical and radiographic evaluation (Fig. 1), the treatment plan involved placing IS+ implants (BTK) in the anterior regions and BT-Rhyno implants (BTK) in the posterior areas using a trans-sinus approach. This strategy aimed to avoid major bone regeneration procedures and reduce overall treatment time.

The procedure was carried out under local anesthesia. A full-thickness flap was elevated to expose the maxilla. In the anterior regions, a conventional implant site preparation protocol was followed. The IS+ implants were placed (Fig. 2) with good primary stability due to the presence of sufficient residual bone.

In the posterior regions, where severe bone atrophy and maxillary sinus pneumatization were present, a trans-sinus approach was necessary. A lateral window was created to elevate the Schneiderian membrane (Fig. 3), a step performed with great care to preserve its integrity. The implant sites were then prepared with an inclined trajectory, crossing the sinus cavity and anchoring into the anterior basal bone. Two BT-Rhyno implants were placed (Fig. 4), specifically designed for trans-sinus placement and to provide stability even in cases of limited crestal bone height.


To complete the implant phase, localized bone regeneration was performed around the posterior implants using autogenous bone harvested with a bone scraper (Fig. 5). This aimed to fill any peri-implant gaps, promoting osseointegration and improving the long-term prognosis.

The surgery was completed in a single session, with minimal invasiveness and respect for soft tissues. The intraoperative images clearly document the key phases of the treatment: placement of anterior IS+ implants, trans-sinus positioning of BT-Rhyno implants, and the use of autogenous bone for final regeneration (Fig. 6).
This combined technique allowed for effective rehabilitation in a complex bone scenario, avoiding more invasive regenerative procedures and enabling a stable and predictable future prosthetic restoration.
