When placing an implant immediately after extraction in the molar region, the main challenge is the discrepancy between the dimensions of the extraction socket and the implant diameter.
In most cases, the implant diameter is too narrow compared to the socket to achieve satisfactory primary stability.
If, on the other hand, complete healing of the post-extraction site is allowed before implant placement, significant contraction of the alveolar bone volume occurs. Bone resorption can reach up to 40% within the first 3–4 months. Following these morphological changes, restoration of the missing tooth typically requires the use of standard or narrow implants, which are not always the optimal biomechanical and anatomical solution.
With this conventional treatment approach, the patient must undergo two surgical procedures: tooth extraction and, after 4–5 months, placement of a standard implant.
